REPORT CRIMINAL ACTIVITY TO DEMOTTE PD!

The following form is anonymous for the purpose of obtaining information from the public.  It does not record your IP address or your e-mail address. This form allows you to submit a tip or information directly to the DeMotte Police Department.   Sometimes though, it is helpful to the department investigator if he has contact information for the person providing the tip.   We may want to contact you further if more information is needed or if something needs to be clarified.   If you choose to provide contact information, it is the policy of the department to keep that information confidential.  If you would like your information to be shared with other law enforcement agencies, please indicate that at the bottom of the form.
